The Medical Assistant supports the Transitional Care Clinic by assisting with patient care, rooming patients, obtaining vital signs, coordinating follow-up care, managing supplies, and supporting patients during their transition from hospital to primary care to promote recovery and prevent readmissions. Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
H. S. Diploma/GED (Required) Responsibilities Performs routine direct patient care duties to assist medical and nursing staff with patient examinations and treatments Maintains patient care environment, equipment, and supplies to ensure patient privacy and safety Rooms patients and prepares them for provider examinations Obtains and records vital signs Collects and documents patient data in N2Q0D Stocks and organizes exam rooms and clinical supplies Conducts patient reminder calls for scheduled appointments Assists with setup and preparation for minor office procedures Supports efficient patient flow and clinic operations Adheres to established safety, infection control, and patient care protocols
