You’ll need
Education
✓High School or GED
Associate or Bachelor’s Degree – Highly Preferred
Qualifications
✓At least two (2) years of experience in HIV treatment, medical case management and/or health education
✓Must have strong written and oral communication skills.
Prior experience using an Electronic Medical Record preferred.
Bilingual English/Spanish or English/Creole preferred

The Medical Case Manager ensures that all PATH adult program patients are referred to appropriate health and psychosocial services, as well as for entitlements.
