Evaluates assessment data to identify nutrition risks, problems, malnutrition criteria, knowledge deficits, and high-risk health behaviors. Offers nutrition recommendations, orders, education, counseling, and referrals to other services. Provides counseling and education that reflect appropriate health literacy and cultural sensitivity to patients with disease processes requiring nutritional management.

Uses evidence-based theories and concepts, including the transtheoretical model and motivational interviewing, in patient education and counseling. Makes follow-up available to detect progress and adjusts nutrition plans of care to reach nutrition goals and promote optimal outcomes. Provides supervised practice experience to dietetic interns and new employees to satisfy learning objectives.

Remains abreast of emerging research applications and evidence-based guidelines, and participates in research, performance improvement, and initiatives. Enters complete, accurate, and timely electronic health record documentation and orders per protocol and standard operating procedure to reflect patient care. Offers timely responses to internal electronic communication.

Participates in patient care rounds and collaborates with physicians, nurses, and other allied health professionals to create nutrition plans of care. Provides expertise to foodservice areas and promotes evidence-based nutrition care. Performs other duties as assigned.
