Description

Provides social work services for Home Health Services (HHS) of SRH. In coordination with physicians, updates care plan based on patient need. Performs evaluation, treatment and implementation of care plan.

Provides referral to community resources and/or for hospitalization. Serves as a resource to health care professionals regarding psychosocial problems and community resources. Performs any other job related duties assigned by Clinical Managers and/or HHS Directors
