The Meds2Beds Pharmacy Technician, under the direction of the Director of Inpatient Pharmacy Services, is an essential member of the hospital pharmacy team dedicated to improving patient care and outcomes by ensuring timely and efficient delivery of prescribed medications to patients at the point of discharge. Operating within a fast-paced hospital environment, this role supports the Meds2Beds program, which bridges the gap between inpatient care and outpatient medication access.
The technician collaborates closely with pharmacists, nurses, case managers, physicians, and other members of the care team to facilitate seamless bedside delivery, thereby reducing barriers to medication adherence and preventing readmissions. Utilizing hospital electronic medical records, pharmacy order systems, and available manufacturer assistance programs, the Meds2Beds Pharmacy Technician is instrumental in improving medication access and helping to lower readmission rates while promoting better overall healthcare outcomes within the hospital.
* Works directly with Case Managers, Nurses, Physicians and other Pharmacy staff to troubleshoot any issues requiring intervention such as prior authorizations or prescription clarifications. * Works with all members of a patient's healthcare team to ensure prescriptions are delivered to the bedside in advance of discharge. * Utilizes the Meds2Beds software to identify potential patients upon admission, explains the benefits of the bedside delivery program, and encourages/facilitates participation by minimizing barriers participation. * Communicates with Southwell Pharmacy and external pharmacies to streamline patient experience and maximize medication adherence. * Identifies payor sources for medications (i.e. insurance coverage, manufacturer copay cards, foundation copay assistance, etc.) * Attends daily Collaborative Care Rounds and other meetings as needed. * Triages and escalates any questions or issues outside the pharmacy technician scope of practice to the appropriate provider (MD, DO, NP, PA, RN, RPh, etc.) * Displays understanding and proficiency with medication reimbursement through third party payors, insurance billing, and the prior authorization process. * Educates providers and patients on Meds2Beds Pharmacy Technician services and how these services enhance patient care. * Serves as the lead contact for patients and providers regarding prescription medication delivery services. * Adheres to all regulatory, safety, and quality standards related to pharmacy practice and medication handling. * Facilitates and participates in the training of new Meds2Beds Pharmacy Technicians. * Serves as a subject matter expert (SME) for Meds2Beds. * Acts as support staff in the pharmacy when needed or when assigned or requested by management. * Supports documentation of medication delivery activities in patient records and assists with reporting to the pharmacy team.
High School Diploma or Equivalent CREDENTIALS: OTHER INFORMATION: Required: * Registered Pharmacy Technician in the State of Georgia * 1 year experience with primary role working with Meds2Beds or equivalent Southwell/Tift Regional Health System, Inc. is an Equal Opportunity Employer.
