Collaborates with the multi-disciplinary team to provide for an on-going assessment of changes in patient status and gathers pertinent patient care data through rounding and patient interaction using all available resources in the inpatient or outpatient setting. Provides crisis intervention on the unit to maintain safety. Responds to facility Code events. (10%) Reports any changes in physical, mental and emotional conditions to nursing staff and suggests modifications to the master treatment plan.
