Licensed Masters Social Work to provide services including discharge planning for acute care population. Inpatient and emergency room assignments. Experienced with acute inpatient, experience discharges planning acute care.
Knowledgeable SNF and home care referrals. Proficient in biopsychosocial assessments. Participate in an innovative inpatient care program providing leadership to a multidisciplinary team.
Geriatric experience. Apply pertinent state and federal regulations regarding social work and discharge planning, familiarity with post-acute provider services, financial screening, and reporting requirements.
Masters in Social Work from an accredited school required. New York State License (LMSW) required. Experienced with acute inpatient, experience discharges planning acute care.
