Nurse Coordinator - Renal Care Center, Vascular Access and Quality

Salary: $198,756.48 - $266,383.52 Annually. Under direction, to assess, plan, implement and evaluate education and to provide other health services for a specific patient population; to function as a consultant and educator to staff. This position requires prior experience in End-Stage Renal Disease (ESRD) care, including coordination of dialysis vascular access, knowledge of hemodialysis treatment processes, and demonstrated experience with Centers for Medicare & Medicaid Services (CMS) Conditions for Coverage (42 CFR 494), Quality Assurance and Performance Improvement (QAPI) programs, and dialysis Plan of Care requirements.
