The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well.
We are seeking a Nurse Manager for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role is responsible for continuity of nursing care by delegating, directing, and monitoring nursing care delivered.
Demonstrates management skills through effective problem solving and decision- making, assuring compliance with State and Federal regulations as well as Ebenezer Quality standards. This schedule includes; 80 hours per two weeks Day shift (Monday - Friday) Transitional Care Unit Weekend rotation will be once every 5 to 6 weeks Responsibilities Leads, supervises, directs and/or delegates all nursing functions on assigned unit or floor to assure that care is provided per individualized plan of care.
Assures staff compliance with State and Federal regulations, Nursing Standards of Practice and Ebenezer Quality Assurance Standards. Responsible for designated unit. Assures that reporting process is accurate, up to date, and provides continuity of care from shift to shift.
Observes and evaluates cares given by nursing staff through daily rounds, random audits (i.e. assures that nursing staff are completing assignments/documentation, for completeness and accuracy). Assures necessary equipment is present (TPN tubing, TF formula, trapeze to bed, oxygen). Assure room placement is appropriate (immune-compromised not placed near infected).
Isolation guidelines are followed if needed. Assures that medications and treatments are administered per physician orders and facility policies. Reviews preadmission assessments as needed.
Delegates tasks/duties to nursing staff within their scope of practice. Monitors mealtimes to assure that: a) appropriate staff coverage and assistance is provided; b) proper diets are served in a timely manner. Evaluates staff performance and address concern promptly and appropriately (staff training or performance issue).
Conducts staff performance reviews & other staff surveys. Coordinates team meetings. Assures that plan of care, medical records and other documentation are up to date.
Coordinates the Admission and Discharge process. Develops and/or assures accuracy of care plan process (RAI process including individualized assessments). Actively participates in IDT meetings and care conferences.
Audits patient records to ensure Code Status and POLST match. Assures that changes in condition or acute episodes are reflected in the medical record/plan of care. Assures that resident family and physician are notified of any changes in plan of care.
Investigates and follows-up on all incident reports, medications errors, and Vulnerable Adult reports. Actively participates in audits to assure quality care and practices. Required Qualifications Vocational/Technical Training or Associate Degree – Registered Nurse 1 year Long Term Care Registered Nurse with the MN Board of Nursing Basic Life Support (American Heart Assoc, Red Cross or MTN) AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor.
American Red Cross: CPR/AED for Professional Rescuers and/or Healthcare Providers, Life Guarding First Aid/CPR/AED
