Position HighlightsMonday-Friday clinic scheduleClinic hours: 7:30 a.m. - 4:00 p.m. Part-time position - 32hrs/weekOutpatient-focused practiceAverage of 8 patients per dayProviders manage their own patient panelMinimal call responsibilities with infrequent urgent patient needsSystem-wide EMRComprehensive benefits including health, dental, vacation, 401k, and CMERelocation assistance and Malpractice insuranceAll patient care is delivered in the outpatient setting, with inpatient involvement limited to communication regarding outpatient referrals.

ResponsibilitiesConduct comprehensive palliative care assessmentsFacilitate goals-of-care and advanced care planning discussionsManage symptoms and improve quality of life for patients with serious illnessesCollaborate with physicians, nursing staff, social workers, and other interdisciplinary team membersCoordinate care transitions and community resourcesProvide compassionate support and education to patients and familiesQualificationsNurse PractitionerMaster's or Doctorate in NursingBoard Certification as a Nurse PractitionerEligible for Missouri licensurePhysician AssistantGraduate of an accredited Physician Assistant programNCCPA certificationEligible for Missouri licensurePreferred ExperienceCandidates should have a strong level of comfort discussing goals of care and serious illness management.

Experience in hospice, palliative care, oncology, geriatrics, or related fields is strongly preferred.
