Madonna Ptak Morton Plant Rehabilitation Center is a 120-bed, 5-star rated facility located on the campus of Morton Plant Hospital. We specialize in post-acute care and rehabilitation services, including physical, occupational, speech, and recreational therapy. Our mission is to provide superior, patient-centered care that enables individuals to regain independence and return home safely.
BayCare is seeking a compassionate and skilled Occupational Therapist with SNF experience to join our interdisciplinary team. The ideal candidate will provide high-quality, evidence-based occupational therapy services to patients recovering from neurological, orthopedic, cardiac, and complex medical conditions. You will play a key role in helping patients regain independence in activities of daily living (ADLs) and instrumental ADLs (IADLs).
Occupational Therapist
Conduct comprehensive evaluations and develop individualized treatment plans. Deliver occupation-based interventions focused on ADL retraining, transfer training, and functional mobility. Implement preparatory activities such as therapeutic exercise, balance retraining, and cognitive interventions.
Collaborate with physical therapists, speech therapists, nurses, physicians, and case managers to ensure coordinated care. Educate patients and caregivers on adaptive equipment, home safety, and discharge planning. Document patient progress and outcomes in accordance with CMS and facility guidelines.
Participate in interdisciplinary team meetings and contribute to care planning. Maintain productivity standards while ensuring ethical and patient-centered care. Qualifications Current Florida license as an Occupational Therapist (OTR).
Or eligible to obtain. BLS (Basic Life Support) required. Bachelor's degree Occupational Therapy required Minimum of 1 year experience in a Skilled Nursing Facility (SNF) or subacute rehab setting preferred.
Knowledge of CMS guidelines and SNF documentation requirements. Strong clinical reasoning and communication skills. Ability to work effectively in a fast-paced, team-oriented environment.
Experience with neurological, orthopedic, and medically complex patients is a plus.
