✓Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
Qualifications

✓Educate patients and families about disease process and potential side effects
✓Assist patient with treatment decision making; develop and use decision aids as appropriate
✓Attend and support Tumor Boards
✓Reduce emergency room utilization and hospital readmissions through outpatient symptom management
✓Follow patients throughout the care continuum including inpatient admissions in collaboration with inpatient care management resources
✓Support transitions of care and coordination between SKCC sites
✓Provide clinical consultation to the Patient Navigators
✓Educate multidisciplinary colleagues about the role of the Nurseand Patient Navigators, identified barriers, available resources and the navigation process
✓Support program growth and patient experience in all patient interactions
