Outreach Coordinator, Peace Hospital

Assess community behavioral health needs, provide input for facility strategic planning, business development, community outreach and maintain ongoing communication with key facility referral sources to promote information about Peace’s services; provide customer service with coordination of patient care; ability to discuss treatment/recommendations with referral sources. Provide Level of Care Assessments as needed.

Under the direct supervision of the Director Behavioral Health Outreach, this position will provide the following:Reviews referral source data and track trends from referral sources in territoryInitiates face to face contact with referral sources throughout the assigned region by conducting outreach visits to their agenciesMaintains positive referral source relationships in targeted regionsResponsible for processing referral source complaints working directly with the Director Behavioral Health Outreach and appropriate administrative personnel in developing strategies to resolve concerns and improve servicesParticipates in monthly department meetings.

Master’s degree in a human services field (required)

At least 2 years’ experience in a behavioral health setting working independently in marketing, business development or clinical setting (required)

Record of working with or building relationships with other community resources (preferred)
