and Responsibilities As an Outreach Patient Navigator, you will provide critical connection and dedicated engagement, building strong relationships within the community and extending access to our valuable services and resources. Every day you will expertly identify community needs, represent our organization at events, develop and deliver educational presentations, and meticulously establish partnerships to broaden our reach and impact.
The priority goal for the FOCUS (Frontlines of Communities in the United States) Patient Navigator is to provide linkage to care for patients diagnosed with Human Immunodeficiency Virus (HIV), Hepatitis B (HBV) and/or Hepatitis C (HCV), and Syphilis where they can receive appropriate levels of care with the desired outcome being improved health and quality of life for designated patient populations. The navigator will promote patient informed decision making and identify/minimize socioeconomic, environmental, and circumstantial issues that bar access to care and hinder compliance with treatment.
He/she will coordinate care plan adherence activities such as appointment reminders, arranging transportation and coordinating the referral process between St. Joseph's Medical Center (SJMC), community health centers/primary care clinics and social supportive resources. The process involves a high level of interaction with SJMC staff, patients and community health providers to determine optimal care needs and settings and transfer relevant patient health data via a secure web-based system and maintain patient documentation on every patient contact, as well as necessary follow-up to ensure continuity of care.
This individual must have excellent communication skills including the ability to communicate sensitive issues, including sexuality, drug use, and physical and mental health. This position requires the full understanding and active participation in fulfilling the Mission of the Organization. Reduce barriers that keep patients from getting timely treatment by identifying patient needs and directing them to sources of emotional, financial, administrative, or cultural support. When there is an issue that the doctor should know about, explain the situation to the doctor so the doctor knows how to address the situation. If patients have low income, help them apply for hospital discounts or money for food, housing, utilities, or dental care. Serves as liaison between patients and providers to find appropriate care centers and connect with providers.
Promote informed decision making and identify/minimize socioeconomic, environmental, and circumstantial issues that bar access to care and hinder compliance with treatment. Keep the patient and families informed of information and resources while building working relationships, solving problems and supporting patients. Daily face-to-face meetings to learn what factors may present barriers to patients' care.
Job Requirements Required At least two years experience in health care, social service setting including any experience conducting community outreach, case management, or patient focused services. Accredited AA Degree with emphasis in social services, psychology or related health field. Equivalent combination of experience and education will be accepted.
