✓Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire

Certifications
✓This job requires credentials from multiple states.
✓Credentials from the primary work state are required at hire.

✓Basic Life Support from American Heart Association
✓Certified Hospice and Palliative Nurse within 18 months of hire
Education
✓Successful completion of palliative care training within 12 months of hire required, including: pain and symptom management, nutrition and hydration, psychosocial and spiritual care, and hospice.
Qualifications
✓Position is onsite at Holy Cross Hospital
✓Minimum two (2) years of clinical nursing practice experience required.
Two years of experience in hospice and palliative care setting preferred.
Experience in acute care, intensive care, emergency, skilled nursing facility, long-term care or managed care settings preferred.
B.S.N. preferred.

About the role

In general, the Palliative Care Nurse Coordinator is responsible for screening palliative care referrals/ evaluating the appropriateness of palliative care referrals, gathering patient and family data in preparation for the palliative care consult by the palliative care team, coordinating palliative care consults, assessing member palliative are needs and monitoring the quality of outcomes.
