Job Duties: Geisinger Community Medicine has much to offer, allowing you to live well in both your professional and personal life: Forty-minute appointments for our senior patient population Daily huddles with all care team members to review essential information about schedules, patients, resource needs, and coordination of care Pharmacists to assist with patient education and medication management Care managers, health managers, clinic health associates, and coding educators work collaboratively with providers Increased access to care by means of eVisits, patient portals, and Geisinger’s ConvenientCare urgent care clinics Position Details:
