Salary Range: $25.74 - $35.46 Pay rates are determined based on experience and internal equity.
This position reports to the Director of Case Management. Assists patients access evidence-based treatment for substance use disorder (SUD) and community resources. Collaborates with providers and clinical staff in identifying patients needing assistance.
Conducts initial brief assessments. Facilitates appointments for patients at outpatient clinics for addiction treatment. Serve as a resource for patients, caregivers, family, and others.
Assist with access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities. Demonstrates quality and effectiveness in work habits. Treats patients, families, visitors, and staff with consideration and respect.

Position Duties/Responsibilities: Assist with identification of patients with a substance use or co-occurring mental health disorders in the ED and, within inpatient units by monitoring patient tracking systems to screen for eligible patients, checking in with clinicians and nursing staff to receive referrals of eligible patients, and placing signage or materials inviting patients to seek help for substance use in prominent areas of the ED and hospital.
Make contact and establish a positive relationship with patients in the hospital struggling with drug use. Use motivational interviewing techniques to communicate with patients in a respectful, culturally appropriate, non-judgmental manner. Maintain up-to-date information about the effects of various substances, withdrawal symptoms, and treatment options in order to effectively educate and counsel patients.
Promote harm-reduction strategies based on patients’ goals, preferences, and life circumstances. Cultivate relationships with external agencies to facilitate referrals.
Accepts appropriate supervision. Nonjudgmental, energetic, positive, harm-reduction approach to assisting patients with SUD. Ability to use computer information systems and knowledge of word processing.
Interest/proficiency in working with individuals recently released from incarceration, homeless individuals, and other marginalized populations. Ability to interact with patients in a culturally relatable manner. Any additional ability to communicate in languages spoken in the local community is helpful.
Respect for patient confidentiality and privacy. Ability to communicate clearly verbally and in writing. Communicates concerns appropriately using the established chain of command.
Position
High school diploma required. Enrollment in MSW program, with anticipated date of completion within 12 months.
At least one (1) year of clinical/customer service/outreach experience is required; behavioral health experience is preferred.
Basic Life Support (BLS) required. Skills/Ability: Excellent clinical skills. Excellent verbal and written communication skills.
Computer skills required. Exhibits the ability to organize work assignments and follow through with accuracy. Exercises good judgment, demonstrates initiative, emotional stability, tact and poise.
Ability to provide excellent results with minimal direct supervision.
