Will be covering census in one facility; but must be flexible to support in other surrounding home facility centers as needed including but not limited to: New London and Norwich. CT. Train patients and caregivers on PD procedures and self-care Manage a caseload of PD patients, including assessments and care planning Monitor treatment progress, conduct home visits, and provide on-call support Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
Monday–Friday, 8-hour shifts Weekends and holidays off Flexible scheduling to manage your own caseload
RN license in the state of practice Willing to obtain both CT and NY State license (we reimburse) Minimum of 12 months of RN experience Valid driver’s license, insurance, and reliable transportation ADN required; BSN preferred Basic computer skills (MS Word, Outlook) Preferred
