✓Requirements
Licensure
✓State of Oregon pharmacist licensure.
✓Board Certified Pharmacotherapy Specialist (BCPS) certification within two years from date of hire.
Certifications
✓Basic Life Support (BLS) certification obtained within 90 days from date of hire.
Education
✓Doctor of pharmacy degree or a bachelor of science in pharmacy with applicable work experience at the discretion of the hiring manager.
Qualifications
✓PGY-1 residency or four years of previous ambulatory care/clinical patient care practice, along with prior direct experience in patient or customer service interaction in-lieu of residency experience.
