Pharmacist

POSITION SUMMARY The pharmacist is responsible for the safe, and cost-effective selection and utilization of medications for patients in which they serve. The pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The pharmacist provides consultative services and education for physicians, nurses, pharmacy staff, patients and caregivers.

The pharmacist provides prescribed medications according to professional standard and practices for patients from 0 through life span. The pharmacist will serve as a leader of pharmacy services in his or her immediate work area including inpatient areas and satellites, and outpatient services. The pharmacist is responsible for directly supervising support personnel in the immediate assigned area and may act as supervisor in situations as assigned.

The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care. By considering the patient’s disease state(s), age, lab values, and other pertinent information, the pharmacist evaluates medication orders and intervenes when appropriate. As a member of a multidisciplinary team, the pharmacist will collaborate with other healthcare professionals and represent pharmacy in a knowledgeable and professional manner.

In addition to planning and implementing pharmaceutical care, the pharmacist will ensure regulatory and quality measures are followed. Cost-effective utilization of medications will be reviewed by the pharmacist during patient rounds, order entry and while performing medication use evaluations (MUEs). As needed, the pharmacist will provide recommendations and/or literature to prescribers supporting the use of formulary agents.

The pharmacist will also establish and review evidence-based order sets within their practice. Individuals in this position will strive to advance personal and departmental pharmaceutical knowledge. The pharmacist is expected to stay current on literature related to pharmacy practice and to engage others in learning through staff in-servicing and other education as needed.

The pharmacist is encouraged to be an active member of hospital committees including: P&T Committee, Code Blue Committee, Medication Safety, Pain Management, Stroke Council, Infection Prevention, Cancer Committee or other committee as designated by Pharmacy Management. JOB QUALIFICATIONS Education or Formal Training B. S. or Pharm D. degree in Pharmacy.

Graduate of an accredited residency program preferred. Special Qualifications (Licensure, Registration, etc.) 1. Current Licensed Pharmacist in the State of Colorado required. 2.

Board certification preferred. 3. BLS/ACLS preferred. Knowledge, Skill and Ability Able to work well and communicate effectively with all people.

Experience None Specified MATERIALS AND EQUIPMENT DIRECTLY USED Various computers, laminar flow and chemo hood, automated dispensing system. WORKING ENVIRONMENT/PHYSICAL ACTIVITIES 1. Will be required to stand for long periods of time. 2.

May be exposed to hazardous and toxic substances. 3. May be required to work rotating shifts, including weekends, nights, and holidays. 4. Able to lift 30 pounds regularly.

INTERRELATIONSHIPS Cooperates with co-workers, physicians, hospital staff, patients and patient families in support of ENCORE Values. SUPERVISORY DUTIES Supervises pharmacy technicians, interns, residents, and volunteers. ESSENTIAL DUTIES AND RESPONSIBILITIES 1.

Interprets and fills medication orders for patients 0 through life span. 2. Maintains patient profiles. 3. Provides education and counseling to patients and their families about medications. 4.

Monitors drug therapy for interactions, allergies, and appropriateness for patients of all ages. 5. Issues controlled substances and maintains records as required by law. 6. Provides education to nurses, physicians, and other staff as required. 7.

Fills out patient prescriptions. 8. Prepares IVs using aseptic technique. 9. Understands and is able to operate the automated dispensing machines. 10.

Participates in clinical programs provided by the Pharmacy. 11. Participates in formulary control. 12. Answers telephones and window. 13.

Attends and participates in other programs, committees, and functions required by the hospital. 14. Adheres to the laws and requirements of the Federal Government and State. 15. Engages in culture of safety activities such as: participating in continuous quality improvement initiatives, and entering safety events. 16.

Assists in the training and education of all technicians, students, pharmacists, and interns. 17. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. 18.

Participates in all drug therapy management protocols including, but not limited to; pharmacokinetics, renal dose adjustments, LMWH dosing in obesity, Warfarin dosing, and IV to PO. 19. Performs finger stick blood testing at anti-coagulation clinic. 20. Performs other duties as assigned to support quality and patient safety. 21.

Apply problem solving and critical thinking skills to clinical responsibilities. 22. Routinely check e-mails, review and reply when needed. 23. Employees are expected to comply with all regulatory requirements, including Joint Commission Standards. 24.

Is familiar with organization, department and job specific Environment of Care areas, including Life Safety, Utilities Management, Hazardous Materials Communications, Emergency Preparedness, Infection Control and Medical Equipment Failure. 25. Adheres to Standard Precautions as appropriate a) The use of protective barriers as appropriate; *gloves *masks *gowns *pocket masks *safety glasses b) Handling and disposing of infectious waste appropriately. c) Hand washing as appropriate.

OTHER DUTIES/RESPONSIBILITIES 1. All employees are expected to remain flexible to meet the needs of the hospital, which may include floating to other departments to assist as the patient needs fluctuate. 2. Other duties as assigned.
