Licensure
✓Licensed [or eligible for licensure (i.e., meets all statutory requirements for Florida licensure application)] as a pharmacist per Florida Statutes (F.S.), Title XXXII (Regulations of Professions and Occupations), Chapter 465 (Pharmacy), Sections 007 through 013.
Advanced training (e.g., residency, fellowship, and/or board certification). preferred
Education
✓Doctor of Pharmacy (Pharm.D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited pharmacy school.
Qualifications
✓Clinical pharmacist.
✓Precepts or provides advanced DI education and training to interprofessionals, pharmacy students, and residents.
✓Two (2) years of hospital pharmacist experience.
✓Human Resources (HR) may accept the successful completion of a postgraduate year two (PYG2) DI pharmacy residency in lieu of the required hospital pharmacist experience.
