Monitor and manage medicationMonitor and manage medication therapy for assigned ambulatory (and occasional inpatient, as needed) patients to ensure optimal, cost-effective outcomes
Communicate with prescribing providersCommunicate with prescribing providers to resolve medication-related issues and document therapeutic interventions
