The Transitions of Care (TOC) Pharmacist is responsible for improving medication safety, continuity of care, and patient outcomes throughout the healthcare continuum. This role collaborates with physicians, nurses, care managers, case managers, ambulatory providers, and community pharmacies to ensure accurate medication reconciliation, patient education, and seamless transitions between inpatient, outpatient, and post-acute care settings.

The TOC Pharmacist identifies and resolves medication-related problems, reduces avoidable readmissions, promotes adherence, and supports organizational quality and population health initiatives.

Medication Management and Care TransitionsPerforms medication reconciliation and medication reviews across admission, transfer, discharge, and post-discharge transitions to reduce medication-related issues. Identifies, resolves, and documents medication-related problems, discrepancies, and adverse drug events. Optimizes medication therapy in collaboration with providers and care teams.

Coordinates prescription fulfillment and facilitates access to medications by addressing affordability, coverage, and adherence barriers. Patient Education and Follow-UpProvides individualized medication counseling and education to patients and caregivers using effective communication and teach-back methods to improve medication adherence and satisfaction. Conducts post-discharge follow-up to assess adherence, medication effectiveness, and patient concerns.

Develops educational resources and supports learner development, as applicable. Clinical Consultation and Interdisciplinary CollaborationParticipates in interdisciplinary rounds, discharge planning, and care coordination activities. Provides evidence-based medication recommendations and monitors high-risk medications and patient populations.

Serves as a liaison across inpatient, outpatient, post-acute, specialty pharmacy, and community care settings to support seamless transitions to improve patient outcomes. Quality Improvement and Population HealthSupports initiatives focused on medication safety, readmission reduction, value-based care, and population health. Analyzes performance data, monitors quality metrics, and participates in regulatory, accreditation, and quality improvement activities.

Assists in developing and improving transitions-of-care workflows, policies, and protocols. Performs other duties as assigned.

Doctor of Pharmacy (PharmD) degree from an ACPE-accredited college or school of pharmacy required; PGY1 Pharmacy Residency plus PGY2 Residency in Ambulatory Care, Internal Medicine, or a related specialty preferred. Current pharmacist license in good standing required. Advanced clinical pharmacy knowledge and application skills required.

Strong knowledge of medication reconciliation, medication therapy management, and discharge medication processes required. Minimum of two years of clinical pharmacy experience in transitions of care, ambulatory care, acute care, or related practice settings preferred. Experience with population health management initiatives preferred.

Certification in Medication Therapy Management (MTM) preferred.
