• Down Coordinator to guide patients along their journey from inpatient to outpatient care. The PHP/IOP Step
• Down Coordinator is responsible for meeting with all eligible patients to discuss the benefits of Peachford's outpatient services as part of their continuum of care treatment. The Step
• Down Coordinator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes. A successful candidate will have a thorough understanding of the step-down processes and ensuring patients have a good understanding of the benefits of outpatient services. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred. Qualifications

Bachelor’s Degree required. Mental health/behavioral sciences field or equivalent preferred.

• A minimum of three (3) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
