Physical Therapist - Home Health - FT

Under general supervision, plans and administers medically prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts. Delegates physical therapy treatment programs to Physical Therapy Assistant per department policy. Responsible for plan of care performed by Physical Therapy Assistant.

Duties are performed within the patient’s home or place of residence and within the home health department.
