About the role

The Physical Therapist will be responsible for establishing and maintaining a comprehensive , individualized interdisciplinary plan of care under Physicians’ orders, in an acute care setting, which includes performing evaluations, integrative treatment planning, setting patient-specific functional goals, performing follow-up assessments, and ongoing patient/ family education; Coordinate patient’s Plan of Care with other Medical and Rehabilitation Staff; Initiate, manage and participate in case conferences, utilization review meetings, and discharge planning in collaboration with Case Managers, establishing continuum of care post-discharge; Provide Physical therapy practice act specific high quality of care and maintain EMR in accordance with Physical Therapy departmental policies and procedures, TJC/CHQ standards, Federal and State regulations, Medicare, Medi-Cal, and primary insurance standards; Foster a constructive and supportive approach to department operations, understanding economic constraints and practices designed to conserve resources, promote professional and collegial relationships; Participate in Process improvement and Quality improvement measures; Provide appropriate supervision and support to Physical Therapy Assistants; Act as a clinical educator for students and other clinical staff.
