PRN / Hourly Range - $37.24 - $55.86 Job Overview: Evaluates, administers and directs administration of therapeutic procedures utilized in Physical Medicine and Rehabilitation, establishes Physical Therapy plans of care and goals; re-assesses patient’s progress at regular, timely intervals, supporting findings with appropriate documentation. Required: Certifications/ License/
IL Active Physical Therapist License / BLS within 30 days New graduates acceptable Bonus: Potential for a performance-based bonus, subject to eligibility criteria and achievement of defined metrics. Details regarding bonus eligibility and performance goals will be provided during the interview process or upon hire. Incentives: Eligible for evening/night, weekend shift incentive based on company policies and applicable job codes.
