You’ll need
Licensure
✓Must hold or be eligible to receive a Physical Therapist license in the State where services will be provided.
✓State licensure must be obtained within the first 90 days of employment.
✓Must maintain all certifications and license according to the expiration date.
Certifications
✓Must be certified in CPR or must become certified within the first 90 days of employment.
✓Addendums are required for 0153 Nurse Leader, 0249 Registry RN, 0252 Registered Nurse, 0235 Advanced Registered Nurse, and 0236 Expert Registered Nurse.
Education
✓Must be a graduate of an accredited school of physical therapy, with a physical therapy degree as follows: Minimum of a Bachelor’s degree, if graduated prior 1/1/2016
✓Doctorate of Physical Therapy required, if graduated after 1/1/2016
✓International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelor’s degree in Physical Therapy.
✓Work experience not required other than clinical experience within the educational training program.
Doctorate of Physical Therapy degree preferred.
Qualifications
✓Must be able to lift 50 pounds, combined lift 100 pounds and push 300 pounds. Must have good verbal and written communication skills. Must be willing to work at other locations as assigned.
