Physical Therapist (PT),Home Health Help patients rebuild strength, mobility, and confidence in the place they feel most comfortable: home. Elara Caring is seeking a compassionate Physical Therapist to serve patients throughout the New Haven territory. In this field-based role, you will provide personalized, one-on-one therapy while helping patients improve their safety, independence, and quality of life.

Meaningful, one-on-one patient care Independence in the field Supportive and collaborative clinical team Fleet vehicle program, restrictions apply, or mileage reimbursement Medical, dental, and vision insurance Pet insurance 401(k) with employer match Paid time off and paid holidays Tuition reimbursement for full-time employees Free continuing education opportunities Career development and advancement opportunities Family and pet bereavement benefits What You’ll Do Evaluate patients’ mobility, strength, safety, and functional needs Develop and implement individualized treatment plans Provide skilled physical therapy services in patients’ homes Help patients improve mobility and safely perform daily activities Educate patients and caregivers on exercises, equipment, and safety techniques Monitor patient progress and adjust treatment plans as needed Collaborate with physicians, caregivers, nurses, therapy professionals, and other members of the care team Communicate changes in patient condition to the appropriate clinical team members Complete accurate and timely clinical documentation Supervise therapy assistants and support staff, as appropriate Support patients in reaching their highest possible level of independence What You’ll Bring Graduate of an American Physical Therapy Association-approved physical therapy program Current and unrestricted Connecticut Physical Therapist license Minimum of one year of experience in a healthcare facility Strong communication, organization, and patient-care skills Ability to work independently in a home-based environment Willingness to travel throughout the assigned Norwalk and Fairfield territory Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $100,000 to $115,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
