You’ll need
Licensure
✓Physical Therapy Licensed or eligible for Physical Therapy License, in the state of Wisconsin.
Certifications
✓American Board of Physical Therapy Specialties board-certifications preferred, but not required.
✓Current BLS of Health Care Providers certification from the American Heart Association or American Red Cross.
Education
✓Obtained, or eligible to obtain, a degree (or equivalent) in Physical Therapy from an accredited Educational Institution.
Doctor of Physical Therapy (DPT) degree preferred.
Qualifications
✓Ability to read and communicate effectively in English.
✓Strong written and verbal skills.
✓Basic computer knowledge.
✓Knowledge of Physical Medicine, Physical Therapy Modalities, anatomy, and physiology.
✓Decision making and problem solving skills.
✓Team building skills.
✓Driver’s License may be required depending on location of hire.

About the role
The PT will be able to identify patients/customers: Inpatients, Outpatients, Home Health/Hospice patients, Sub-Acute Unit patients (neonates to geriatrics). The PT is responsible for performing an age-specific assessment designed to gather data relevant to the patient's individual needs. This includes assessment and evaluation of cognitive, sensory, neurological, musculoskeletal, functional, and physiological status.
