Job Summary and Qualifications The Physical Therapist (PT) provides high quality, patient-centered care by managing the complete process of therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The Physical Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience.
The Physical Therapist provides or directs a majority of the patient’s physical therapy to support pati ents throughout the continuum of care. What qualifications you will need: Required: Bachelor’s degree in Physical Therapy Currently licensed as a Physical Therapist in the state(s) of practice, in accordance with law and regulation.
