Physical Therapist

We are looking for an experienced Home Care Physical Therapist. This is a per diem position working as needed. The Home Care office is located in York, Maine.

The Physical Therapist is responsible for: Providing physical therapy services to patients in their home in accordance with a written Plan of Care approved by a physician. The Physical Therapist makes an initial evaluation and plans with patient, and physician for on-going physical therapy services. Initiates, reviews and implements the PT Care Plan for each patient based on the assessment and prescribed Plan of Care and revises when indicated.

We need candidates with these

2 or more years of Physical Therapy experience, exceptional customer service and computer skills, State of Maine PT License, Basic Life Support Certification and a valid driver's license.
