Physical Therapy Assistant, Home Health, Resource Pool, Weekends

Provides physical therapy (PT) support services to the Home Health Services (HHS) physical therapy team for patients by means of following assessment, planning and implementation of treatment. Remains proficient in implementation of clinical procedures such as therapeutic activities, modalities, adaptations, and patient and family education and assessment of durable medical equipment (DME) needs. Maintains personal professional growth through continuing education and independent study.

Fosters open communication and rapport with other disciplines, physicians, all hospital disciplines and outside agencies as needed to ensure patient needs are met. Completes all documentation per Home Heath policies. Performs all other duties as assigned by HHS Managers or Directors.
