P. of Michigan Deliver Patient-Centered Care Beyond the Clinic Walls Bellin Health is seeking a Physician to join our innovative My Home My Care program , serving patients across Escanaba, Iron Mountain, Peshtigo, Green Bay, and surrounding communities . This unique role offers the opportunity to provide high-quality, relationship-based care directly in nursing homes, assisted living facilities, and select home settings .
This is an ideal opportunity for a physician who values autonomy, meaningful patient connections, and a proactive approach to care delivery outside of the traditional clinic setting. Position Highlights Full-time (1.0 FTE), Open to part-time 80% Nursing Home 20% Assisted Living/home Mobile, community-based care model Provide on-site care in partner facilities and select patient homes Strong interdisciplinary team support Practice Details Deliver transitional, acute, and routine care in: Nursing homes Assisted living facilities Select private residences (case-by-case) Practice operates as a mobile clinic model , bringing care directly to patients Focus on managing chronic conditions and complex patient needs Collaborate with a multidisciplinary care team to provide proactive, coordinated care This model enhances patient outcomes by eliminating barriers to access and ensuring continuity of care in familiar environments.
