You’ll need
Licensure
✓Current unrestricted medical license in Indiana
✓Unrestricted DEA license for state of Indiana
Certifications
✓DOT/FMCSA certification - active or willing to get during credentialing
Education
✓Medical degree (MD) or Doctor of Osteopathy (DO) degree from accredited institution
Qualifications
✓Must be eligible to participate in Medicare
✓Job-Related Skills/Competencies
