Benefit Eligible: Yes Population Health Coordinator Cherry County Hospital & Clinic | Valentine, NE Join our team at Cherry County Hospital & Clinic and help improve patient health through preventive care, chronic disease management, and quality improvement initiatives. What You’ll Do Identify and track patient care gaps, including screenings, immunizations, and chronic disease follow-ups. Contact patients to schedule recommended services and follow-up appointments.
Monitor quality measures and help improve patient outcomes. Support value-based care programs and payer quality initiatives. Maintain patient registries, review reports, and track performance data.
Collaborate with providers, nursing staff, and leadership to improve workflows and quality of care. Educate patients about preventive services and available resources. What We’re Looking For Healthcare quality, population health, care coordination, or clinic experience preferred.
Understanding of quality measures and value-based care preferred. Strong communication, organizational, and analytical skills. Experience with electronic health records and Microsoft Office preferred.
Ability to manage multiple tasks while maintaining accuracy and confidentiality.
