About the role

ESKD patients are affected by more than just kidney disease – they almost always present with a consistent constellation of cardiovascular, kidney, and metabolic diseases, also referred to as CKM. It is estimated that 30-50% of patients with renal failure do not receive consistent primary care or other wrap-around support, resulting in hospitalizations and mortality.
