You’ll need
Licensure
✓Maintain CEUs required for continued medical license in Indiana.
✓Current Board Certification in Internal Medicine, Family Practice, or Pediatrics, or eligible to be Board certified.
✓Prescriptive Authority in Indiana (DEA License).

Certifications
✓CPI (Crisis Prevention Intervention) - certification must be obtained after hire and prior to providing direct patient care.
✓This certification must be maintained and kept current through employment.
✓BLS (Basic Life Support) and First Aid - certification must be obtained after hire and prior to providing direct patient care.

✓D., D.O. required.
✓Doctorate (required)
Qualifications
✓Collect, record, and maintain patient information.
✓Prescribe or administer treatment, medical therapy, medication, vaccination and other specialized psychiatric medical care to treat or prevent illness, disease or injury.
✓Order, perform and interpret tests and analyze records, reports, and examination information to diagnose a patient's condition.
✓Monitor patient's conditions and progress and reevaluate treatments as necessary.
✓Explain procedures and discuss test results or prescribed treatments with patients.
✓Refer patients to medical specialists or other practitioners as necessary.
✓Valid driver’s license and active auto insurance coverage.
✓Satisfactory results from criminal and driving record background checks.
