Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel.

Monday – Friday, 0800-1630 No nights, weekends, or holidays! **Must complete three months of on-site training prior to transitioning to a hybrid work-from-home schedule. After training, this role follows a 50/50 hybrid model, with full-time (1.0 FTE) co-workers expected to work on-site five days per pay period. Must also be available to attend monthly on-site clinic meetings.** Highly Preferred
Proficiency with Epic EMR system. Timely and thorough patient triage skills. Computer competency (most of this position will be computer work).
Excellent time management and prioritization. Adaptability in a new and changing environment. LPN
Diploma in Practical Nursing.
Active LPN license.
2-3 years of experience preferred. RN
Associate of Applied Science in Registered Nursing, Graduation from registered nursing program, bachelor's degree in nursing (BSN), OR 4 years of commensurate experience in lieu of the bachelor’s degree.
Active RN license.
