The case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned.

Required
1 year Job related experience in a healthcare setting.
Associate's Degree Bachelor's Degree Upon hire: Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology Prior experience in discharge planning, billing, utilization review, financial counseling or registration. Proficiency in medical terminology.
