The Quality and Patient Safety Analyst is responsible for reviewing video footage following serious safety events, patient harm incidents, security events, and other high-risk occurrences to support root cause analysis, performance improvement, and risk reduction efforts. This position will also conduct proactive random surveillance reviews to identify process deviations, environmental hazards, and opportunities to improve patient and staff safety.
This position serves as a key member of the Corporate Quality & Patient Safety team and partners closely with Risk Management, Security, Nursing Operations, Clinical Leadership, and Human Resources.
Conduct video-based investigations of serious patient safety events, security incidents, employee injuries, workplace violence, patient elopements, and other high-risk occurrences. Analyze video footage and related documentation to develop detailed event timelines, identify key contributing factors, and support comprehensive incident investigations. Correlate video findings with medical records, incident reports, security reports, and other sources to establish an accurate understanding of events.
Provide objective, fact-based observations and analysis to support Root Cause Analyses (RCAs), safety event reviews, risk assessments, and performance improvement initiatives. Identify process failures, human factors, environmental hazards, communication breakdowns, and policy compliance issues that may contribute to adverse events. Prepare clear, concise investigative reports and present findings to leadership, safety committees, and multidisciplinary review teams.
Conduct proactive surveillance reviews to identify patient and staff safety risks, operational vulnerabilities, process deviations, and opportunities for improvement. Analyze trends and recurring patterns across incidents, locations, shifts, and operational areas to support targeted risk-reduction strategies and organizational safety goals. Maintain the security, confidentiality, integrity, and proper retention of video evidence in accordance with HIPAA, organizational policies, and regulatory requirements.
Collaborate with Security, Quality, Patient Safety, Risk Management, Legal, and operational leaders to support investigations, regulatory reviews, and continuous safety improvement efforts. Excel Pivot Tables, etc.) Work Environment: Subject to many interruptions. Occasional pressure due to multiple calls and inquiries. This position can be high paced and stressful; must be able to cope mentally and physically to atmosphere.
Work requires spending approximately 90% or more of the time inside a building that offers protection from weather conditions but not necessarily from temperature changes. Remote position requiring confidential HIPAA compliant workspace. Required to have access to audio and camera capabilities via computer for meeting and other work relatated duties as assigned.
