✓Extensive knowledge of Medication Safety, Joint Commission, Board of Pharmacy, Department of Health, DEA, CMS, Joint Commission, and other regulatory agencies.
✓Active and valid Pennsylvania Pharmacist license or eligibility required.
Certifications
ASHP/ISMP Medications Safety Certificate preferred
Additional certifications include Lean Six Sigma, Certified Professional in Patient Safety (CPPS) preferred
Education
✓Bachelor's degree-BS Pharmacy or PharmD required, Master's degree preferred.
Qualifications
✓Minimum of five (5) years of pharmacy experience in a health system setting.
✓Minimum of five (5) years of pharmacy experience in Medication Safety and Joint Commission.
✓Training and/or experience in medication safety, quality improvement, performance improvement, regulatory and accreditation compliance required.
PGY-1 Residency preferred
