The registered dietitian provides medical nutrition therapy to ambulatory patients individually or in a group setting. The registered dietitian provides a nutrition assessment, determines and implements a nutrition care plan and intervention. Qualifications Education Level Field of Study Bachelor's Degree Licenses/Certifications Licensed Dietitian Nutritionist - Illinois Department of Financial and Professional Regulation (IDFPR) And Registered Dietitian (RD) - Commission on Dietetic Registration (CDR) Certified Diabetes Care and Education Specialist preferred - Achieve and maintain qualifications for level as outlined in the ladder pay practice. Provide nutrition services and medical nutrition therapy integrated with the patient's medical goals. Identify and provide age specific nutrition counseling to meet the cultural needs of the patients and their families Evaluate, interpret, monitor and document the nutritional status and nutritional needs of patients using established standards of care and practice guidelines. Develop, review, update and implement educational materials to meet the needs of patients and professionals Department Specific Job Function Completes clinical and educational assessment on all patients.
Develops individualized teaching plans for self-management (Diet, Medication, Self-care) Implements evidence based clinical practice guidelines for all area of nutrition education Provides education to all patients utilizing insulin pump. Collaborates will all members of the health care team
