Licensure
✓Current Registered Nurse (RN) license in state of practice.
✓Valid compact licenses must be transferred to the state of practice within 60 days of the start of employment.
Certifications
✓Basic Life Support Certification (BLS) for Healthcare Providers.
✓Unit specific certification as required (i.e., ACLS, PALS, etc.).
Case Management Certification preferred
Education
✓Bachelor of Science in nursing (BSN) from an accredited institution (degree verification required).
Qualifications
✓RNs must obtain their BSNs within four (4) years of hire or promotion date. If there is an existing education agreement, that agreement will take precedence.*
✓Current driver’s license, insurance, reliable transportation, and an acceptable driving record.
✓Demonstrated clinical nursing experience.
✓Demonstrated knowledge of home health or hospice.
✓Ability to communicate effectively both verbally and in writing.
One (1) year experience working in home health or hospice. preferred
Three (3) years of clinical nursing experience. preferred
Experience working with third-party payors preferred
Previous experience in case management, utilization review, and/or discharge planning. preferred

About the role
