In this role as a Dialysis RN - Peritoneal Dialysis , you will educate patients and caregivers in-clinic, enabling them to safely and independently perform home peritoneal dialysis. Key Responsibilities As a Dialysis RN - Peritoneal Dialysis (PD) , you will: Train & Educate: Conduct one-on-one training with patients and caregivers on Peritoneal Dialysis procedures, sterile techniques, and home self-care routines. Case Management: Manage a dedicated caseload of peritoneal dialysis patients, performing ongoing clinical assessments, individualized care planning, and progress tracking. Clinical Oversight: Monitor home treatment adherence, conduct routine clinic check-ins and occasional home visits, and provide dedicated on-call clinical support. Interdisciplinary Collaboration: Partner with nephrologists, dietitians, social workers, and clinical managers to drive safe, high-quality patient outcomes. Schedule & Work
