About the role

In this flexible on-call role, you'll provide in-home skilled nursing visits, complete Start of Care admissions as needed, and deliver exceptional patient-centered care while being supported by an experienced clinical team.

What you’ll do
Respond to after-hours patientRespond to after-hours patient needs within your assigned coverage area
Complete Start of CareComplete Start of Care (SOC) admissions as needed
Perform comprehensive patient assessmentsPerform comprehensive patient assessments and complete OASIS documentation
Deliver skilled nursing careDeliver skilled nursing care, including medication administration, wound care, treatments, and other prescribed interventions
Monitor patient conditions andMonitor patient conditions and communicate changes to the Clinical or Client Services Manager
