✓Requirements
Licensure
✓Current Registered Nurse (RN) license in state of practice.
✓Valid compact licenses must be transferred to the state of practice within 60 days of employment.
Certifications
✓Basic Life Support (BLS) Certification for Healthcare Providers.
Certification in Case Management, Care Coordination, or a relevant specialty area. preferred
Education
✓Bachelor of Science in Nursing (BSN) required or must be obtained within four (4) years of hire or promotion date.
Bachelor’s degree in nursing (BSN) from an accredited institution. preferred
Qualifications
✓If there is an existing education agreement, that agreement will take precedence.
✓Demonstrated expert medical surgical nursing experience.
✓Proven clinical leadership experience, such as serving as a charge nurse, preceptor, or in a formal mentoring capacity.
✓Proven ability to coordinate complex care, facilitate interdisciplinary collaboration, and lead quality improvement initiatives.
✓Demonstrated ability to work independently and exercise a high degree of autonomy, including making complex clinical decisions and prioritizing care in a dynamic, remote environment.
✓Strong communication, critical thinking, and problem-solving skills, with a track record of effective decision-making.
Experience with virtual care delivery, Hospital at Home programs, or advanced care coordination. preferred
