Location: Methodist Estabrook Cancer Center Address: 8303 Dodge Street, Omaha, NE

Availability amongst our two shifts. 0700-1530, and 1500-2330. Minimum 24 hours per month to comply with PRN status. The IV Team Registered Nurse provides specialized services throughout the hospital, responding to requests across all inpatient units.

This role is responsible for the assessment, insertion, maintenance, and management of vascular access devices while promoting patient safety, quality outcomes, and evidence-based practice. Key

Respond to vascular access requests from all hospital departments and inpatient units. Insert and maintain peripheral intravenous (PIV) catheters. Perform ultrasound-guided peripheral IV insertions if this is a known skill.

Insert and manage midline catheters if this is a known skill. Insert and manage Peripherally Inserted Central Catheters (PICCs) if this is a known skill. Access and de-access implanted ports according to organizational policies and procedures.

Perform and monitor dressing changes for midlines, PICCs, ports, and other vascular access devices. Assess vascular access sites for complications, including infection, infiltration, occlusion, and thrombosis, and intervene as appropriate. Provide education and support to patients, families, and healthcare staff regarding vascular access device care and maintenance.

Maintain accurate documentation of procedures, assessments, and patient outcomes in the electronic medical record. Perform other duties as assigned.

1. Provide appropriate care specific to the age of the customer/patient, to ensure understanding and comfort level of treatment as outlined in the Population Specific Criteria. Demonstrate ability to identify opportunities for and give individualized customer service/patient care according to customer/patient needs. 2.

Perform an initial and ongoing assessment to determine the assigned patient's ongoing nursing care needs. Include data from the patient/significant other(s) in the assessment as appropriate. Prioritize data collection according to patient's immediate needs.

Use assessment findings in discharge planning. 3. Develop/review/revise and individualize the plan of care to address the patient's nursing care needs for optimum patient care outcomes. (Consultation and resources, nurses as teachers.) Collaborate with providers and other clinical disciplines as appropriate.

Provide teaching specific to the patient's health care problem. Care plan reflects continuing care needs and/or referrals for discharge/transfer. 4. Implement interventions that are consistent with the established plan of care in a safe and appropriate manner (Professional Models of Care).

Is familiar with, and follows, nursing protocols and standards of care for their unit. Use current knowledge in nursing practice based on standards and research literature. 5. Evaluate patient care to determine response to care/interventions for optimal patient outcome.

Monitor outcomes and documents patient response to interventions. Evaluate the patient's capabilities and/or limitations related to nursing and medical interventions. Continually monitors the patient and/or significant other(s) ability to manage care needs after dismissal.

Give a complete verbal or written change of shift report to the oncoming care provider. Includes current care requirements, to include SBAR (situation, background, assessment and recommendation) content, and is familiar with the overall plan of care. Recognize and address patient and family capabilities and limitations (physical and mental). 6.

Perform technical skills competently and independently to assure patient safety (autonomy). Able to competently perform technical skills independently. Is accountable to learn new skills and equipment within areas' specifications and time frame.

Demonstrate correct and safe technique in the use of equipment according to specific product information and policy and procedure manuals. Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies. 7. Maintain a professional behavior in attitude, demeanor, interactions to achieve organization's mission.

Initiate action to correct, prevent, and/or minimize risk(s) to the patient, family or hospital employees (e.g., falls, needle sticks, patient concerns). (Quality of Care) Complete appropriate report form for any risk (e.g., variance/incident report, patient concern report, maintenance request). Communicate to appropriate personnel regarding unresolved issues.

Work within the scope of practice as defined by State Nurse Practice Act. Observes appropriate precautions when administering hazardous medications or handling excreta from patients who have received hazardous medications. 9. Review, delegate and evaluate plans for appropriate utilization of staff at all levels of practice in accordance with the provision of the state's Nurse Practice Act and the Professional Standards of Practice.

Performs venipunctures for intravenous infusions and blood transfusions. Consistently complies with Standard Precautions while performing venipunctures. Consistently uses aseptic technique while performing venipunctures.

Consistently documents accurately in patient record. Consistently adjusts patient care plan to reflect current status of IV Therapy. Actions taken are consistent with protocol. 12.

Performs an ongoing assessment of assigned patients' IV sites and initiates interventions based on the assessments. Consistently performs assigned daily floor checks to assess sites and accumulate 96 hour restarts due. Consistently restarts all IVs at 96 hours based on assessment of patient situation.

Consistently restarts, redresses or discontinues IVs based on assessments on rounds. Consistently documents on floor check sheets problem sites for further follow up. Actions taken are consistent with protocol. 13.

Performs outpatient blood specimen collections from central venous access devices Consistently accesses central venous access devices (Groshong's, Hickman's, implanted ports) for blood specimen collection in lab and Outpatient Oncology. Maintains current knowledge of different devices and recommended flush protocols. 14. Performs dressing changes for Central Venous, arterial and intra-aortic catheter sites.

Consistently performs dressing changes day after insertion and then every week Consistently practices sterile technique with dressing changes. Consistently documents procedure and assessment of site in progress notes of patient record, and patient dressing change sheet. Actions taken are consistent with protocol. 15.

Assists physicians with Central Venous catheter insertions. Consistently obtains supplies needed for insertion based on patient need and physician preference. Reinforces physician's explanation to patient and prepares patient for procedure.

Assists physician by handing supplies. Consistently documents procedure in patient record, and adjusts the patient care plan. 16. Assesses implanted ports for intravenous therapy and blood transfusions.

Consistently performs using sterile technique. Consistently documents in patient record and care plan to include proper flushing protocol. Actions taken are consistent with protocol. 17.

Provides educational opportunities in intravenous therapies. Preceptor for RNs and student nurses on 1:1 ratio to provide actual skills training of IV therapy. Provides in-service to staff regarding all aspects of IV therapy. 18.

Maintains current knowledge of clinical nursing practice and hospital related policies/procedures/practices. Obtains contact hours of continuing educational offerings. Attends monthly staff meetings on regular basis.

Completes mandatory educational reviews. Maintains current Basic Life Support certification.

• Job Requirements Education Bachelor of Science in Nursing (BSN) from an accredited school of nursing required OR obtained within 5 years from date of hire. Successful completion of annual mandatory education requirements. Experience Requires 3
• 6 months on-the-job training in order to learn proper techniques and special equipment. Experience in venipuncture preferred. License/Certifications Current valid Nebraska Registered Nurse (RN) License, valid compact multistate license, or a temporary permit while awaiting licensure required. Current American Heart Association or American Red Cross Basic Life Support (BLS) strongly preferred at time of hire, required within 3 months of hire. Certification(s) in area of specialty preferred. From National Certification Corporation (NCC) within 1 year of eligibility. Certified Registered Nurse Infusion (CRNI) preferred. Skills/Knowledge/Abilities Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers. Ability to multi-task and prioritize work assignments. Ability to learn, retain, and utilize computer skills that are required for department and hospital information system.
