• Compiles data, tracks outcomes, and makes recommendations for process improvement. Develops and maintains relationships with physicians, post-acute providers, therapists, patients, and families. Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services. Ensures seamless transitions across the continuum of care. Educates patients and families about post-acute care services and expectations.
