Essential Communication: Accurately record, track, and effectively communicate changes in patient condition and progress to the entire interdisciplinary team, ensuring proactive and timely interventions.
Precision and Execution: Flawlessly note and carry out physician and advanced practice provider orders, maintaining adherence to clinical protocols and playing a critical role in the delivery of treatment. Interdisciplinary Orchestration: Act as the primary coordinator for all clinical disciplinesâfrom physical therapy to social work and pharmacyâto ensure seamless, cohesive, and collaborative patient care.
